Test Results Please telephone for any results after 2pm and try NOT to telephone during morning or evening surgery when the staff on the desk are extremely busy. We would appreciate your consideration in this matter.
Church View Surgery & The Jones Family Practice are only able to provide travel vaccines which can be given under the NHS. These include Hepatitis A, Tetanus, Polio and Typhoid.
If you think you require travel vaccines you need to contact a local travel service for advice and a travel risk assessment. You will need to take a list of the vaccinations which you have had. We can provide this for you. Written consent must be given for collecting this on behalf of somebody else.
If any NHS vaccinations are recommended you may book an appointment with our Practice Nurse. If the next available appointment we offer is not soon enough for your travel needs then you will have to find an alternative place to have these done and may need to pay for them.
You MUST bring your travel risk assessment to the surgery at least 7 working days before the appointment so that we can add a copy to your record. If we do not receive this your appointment will be cancelled. You will not receive any vaccinations without this.
The surgery will not accept any responsibility for any costs incurred for the travel risk assessment. This is strictly at the patient’s expense.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
Unfortunately there can sometimes be a shortage of stock in the UK for travel vaccinations available within the NHS, please check with the surgery for further details.
We are able to provide full and confidential advice on contraception, menopause and sexual health screening.
The service includes but is not limited to:
Contraceptive injections (Depo Provera)
Mirena Coil checks and removals
Contraceptive Pill advice
If we are unable to offer a service which you require you can call the Essex Sexual Health Service number on 0300 003 1212 and they will tell you the closest clinic to that offers the service you require and book you an appointment.
Cervical screening is available for women aged 24 1/2 to 65 years
Patients will receive a letter advising them that their cervical smear test is due and they should book an appointment with the practice nurse.
The Practice Nurse can advise you how long you should expect to wait for your results. If after this time you still haven't received them, please contact the surgery.
We offer NHS Health Checks to patients between the ages of 40-74 to help prevent diabetes, heart disease, kidney disease, strokes and dementia.
For patients 75 years and over we also offer a senior health check.
We can offer dietary advice and weight management.
Smoking Cessation clinics are also available within the surgery.
We have a blood pressure machine at each site within the waiting room for patients to use.
Our nurses and healthcare assistants are able to offer wound management and suture removal.
Our GPs are trained to offer advice and guidance on chronic diseases such as Asthma, COPD, Hypertension, Diabetes and Heart Disease. As well as other long term conditions.
Children's immunisations are performed by our practice nurses under the NHS Child Health Surveillance. Usually patients are sent appointments by the Health Authority when the child is due for immunisation. However, if such appointment is not received, parents can contact the surgery to make their own appointment.
Parents are welcome to speak to the Practice Nurse if they have any concerns regarding any of the recommended immunisations.
The current recommended schedule is:
Aged 8 weeks Diphtheria, Tetanus, Whooping Cough, Hib, Rotarix, Polio (Pediacel), Pneumococcal (2 injections) and Men B.
Aged 12 weeks Pediacel, Rotarix and Meningitis C (2 injections).
Aged 16 weeks Pediacel, Meningitis C and Pneumococcal (3 injections), Men B.
Aged 12 - 15 months Meningitis C/Hib (1 injection), Mumps, Measles, Rubella, German
Measles (also called MMR) and Pneumococcal (2 injections),
Men B Booster.
Aged 3 1/2 - 5 years Booster of Diphtheria, Whooping Cough, Tetanus, Polio (prior to
school entry) plus second MMR dose.
School Year 8 HPV
Aged 13 - 18 years Diphtheria, Tetanus and Polio Booster, Men ACWY
We are able to offer the service of Minor Surgery and Joint Injections.
Minor surgery procedures include skin wart and lump removal where necessary under local anaesthetic. This has led to early detection of skin cancer. For non cancer skin conditions this can avoid any continuing nuisance caused over a period of time.
Joint injections are now routinely given for shoulder, elbow, wrist and knee conditions, carpal tunnel syndrome, painful heel conditions and other inflammatory conditions etc.
Please ask at reception for further information. Booking for these procedures is short and straightforward.
With the help of Dr Mahmud we have developed a structured team of clinicians who treat our patients as they are treated in the hospitals. This means that newer medicines which were only used or initiated in the hospital by consultants in Diabetes in the past, are now used in our surgery by our clinicians.
In the past there were difficulties with using Insulin in some patients and now there are suitable alternatives in the right context for some patients which we can apply.
We offer diabetic checks by our healthcare assistants and nursing team.
