Sick (fit) notes

If you require a sick note (fit note), you can submit your request through Anima Admin Triage. Please provide as much information as possible about your illness, including the dates you need the fit note to cover.

Please note that for the first 7 calendar days of illness, you can self certify your sickness with your employer and do not usually need a doctor’s fit note. If your illness lasts longer than 7 days, a fit note may be required.

Fit notes are issued only when clinically appropriate and may require a clinician to assess your condition. If your symptoms have changed or worsened, please include this information in your Anima request, as you may need an appointment before a fit note can be issued.
